1. State the name of church or school group.
2. List the trip coordinator or speaker.
3. State the location of the evangelism meetings.
4. Give the date of the meetings.
5. Include the number of people on the team.
6. Describe the type of mission work completed, i.e. evangelism, health expo, construction.
7. Describe what your team did.
8. What were some of the results of your activities?
9. Include special conversion stories involving the native people who came to the meetings.
10. What type of children’s program was conducted during the meetings? Describe the program and the children who came.
11. What type of village outreach, i.e. door-to-door visitations, VBS, health clinics, Bible studies, were conducted during the day, prior to the meetings?
12. Who was impacted as a result of the village outreach programs?
13. If you have any answered prayers that occurred during your short-term trip, please describe them.
14. What was the most unusual event that occurred during your short-term mission trip?
15. How much money did your team raise for their trip?
16. How did your team members raise the money for this trip?
17. What sacrifices were made by team members to go on this mission trip?
